This program is for anyone with a long term health condition. Meet together with trained leaders for two and a half hours a week for six weeks to try new ways of managing: one small step at a time. It's about easy, everyday things that can help, including ways to deal with problems such as: - frustration, fatigue, pain and isolation - appropriate exercise, nutrition and use of medicines. - communicating effectively with family friends and health professionals - use of community resources
