Community Aged Care Packages are flexible packages of care for people with complex needs who want to remain in their own homes. Services provided may include personal care, meal preparation, respite, social support. An ACAT assessment is required to be eligible for packages. Referral is via The Aged Care Assessment Team (ACAT) at Port Kembla 1300 792 755 The program can provide Indigenous workers.
