Range of nursing services provided in the home following assessment of persons needs, including wound management, continence care and nursing care. Nursing service for frail aged & people with disabilities, as well as general public. Includes advocacy, palliative care, advice on health issues & hearing clinic. Home visits are available (first visit) for parents with a new baby who live within the Shire. Services for children are available at Early Childhood Centres or Community Nursing Clinics in Primary Schools. Referral through self, family, medical or agency. Referrals from other agencies through the CIARR form. Palliative care & in-home nursing require an in-home interview with clients & their carers.
